style: improve markdown rendering output formatting

- Use terminal-link for clickable hyperlinks
- Reduce heading color brightness
- Remove extra trailing newlines from code blocks
- Fix horizontal rule spacing
- Wrap codespan content in backticks
- Suppress language error warnings from highlighter
